Jayme Sankey of Grovetown correctly guessed that it is Crest Be Toothpaste (4.5 oz). CVS has it for $4.99 with your Extra Care Card. Use the printable $1 off coupon, plus get back $3.25 in Extra Care Bucks, and it's like getting the toothpaste for $.74 plus tax.
